Question ratings

The Question ratings form asks customers to rate multiple questions instead of choosing category-based experience indicators.

You need AdaptiveSurvey - Configure project settings permission for the project.

  1. Open AdaptiveSurvey.
  2. In the top right corner, select Configure.
  3. In the Survey form tab, select Question ratings.

Use the question controls to:

  • Edit question labels and prompts.
  • Add questions.
  • Reorder questions.
  • Show or hide questions.
  • Delete questions that should not appear in future surveys.
  • Restore deleted questions from Deleted questions.

The form can have up to 8 visible questions. At least one question must stay visible.

Question labels must be unique.

Open Survey settings to edit the customer-facing copy for the selected form.

Review and update:

  • Rating style: choose Numbers or Stars.
  • Lost Time wording when Lost Time is enabled for this form.
  • Title and intro text.
  • Comment label and placeholder.
  • Submit button text.

Use Survey preview to confirm how the form will look before customers receive it.

  1. Answer the required questions to check the selected requirement rule.
  2. Check the rating style, wording, comment field, and submit button.

Select Save before leaving Configure. Survey form changes are project-specific.

Use Restore defaults if you want to return the selected form to the default values.

Continue to Automation setup to set up Email or Slack delivery.
